All student athletes must have a completed physical form on file before participating in tryouts or practices. Physical forms are due to your school by November 12, 2025 All physicals are performed at the Franklin County Health Department by Nurse Practitioner Susan Moore, who brings over 30 years of experience in family care, men’s and women’s health, immunizations, and more.
